A copy of the applicant's full clinic card (This must reflect all the vaccines administered, including the 4 - 6 year vaccine)

This must include the front page of the clinic card that reflects the applicant's name, as well as all the pages with the records of his vaccinations.

Foreign Applicant Requirements

A certified legible copy of the applicant's passport (depicting his picture and the entry stamp to South Africa).
A certified copy of a valid study permit (if applicable) or parent's work permit.

A letter and report from the first school attended in South Africa (if applicable).

Your information will only be used for the admissions process, and will be retained for a period of 10 years, after which it will be destroyed. Should you refuse or withhold your consent for your information to be used for these purposes, in relation to the admissions process, this will constrain the School’s ability to process your son’s application to Maritzburg College.
